REFERRAL PROGRAM.
If you would like to enrol in our referral program, please fill out the form below. Once approved, you will be sent a registration code, which you can use at checkout to redeem your $50 discount!
Please note that in order to be eligible for the program, applicants must meet the following requirements:
Must be a first-time participant in any Momentum basketball programs. This includes external after-school and club programs.
The information you send in with your application must include the name of the parent who referred you.
Only one code can be redeemed per semester, both for new applicants and referring parents.
